If you have lost your original policy documents, you can apply for duplicate Policy Documents by submitting:
- Affidavit & Declaration for Issuance of Duplicate Policy Documents
- Indemnity Bond For Issuance of Duplicate Policy Documentsalong with processing fee and stamp duty charges as applicable.
